As an RN over the years I have seen CDRP consults ordered, but it is usually for someone with intractable cancer pain, or a person that has a very high tolerance to narcotics secondary to being on them forever due to chronic back pain or other chronic problems that require chronic pain management. I have seen CDRP consults for drug abusers because these people do not get pain relief as you or I would after surgery, because they have such a high tolerance to narcotics. I think sometimes the surgeons become frustrated because the doses are just to high. These pts should have strict vs parameters and be monitored on tele, and followed by CDRP.

I think this is just another reason to make sure California keeps the CURES website and I support Attorney General Harris for keeping the site alive and hope she is able to get the funds to continue the site. @Jill Smith: I couldn't agree more and that is exactly the type of problem I'm talking about. WOW! Vicodin for cramps, let alone a 13 year old! I saw a patient just the other day who has severe back pain and wanted me to prescribe her stronger opiates than she was already on. However, her MRI was completely unremarkable. She was also 320 pounds and 5'4". I also see patients for chronic pain from cancer and for those patients, I have no problem giving them whatever they want. And agreed also, so many post-op patients require so much following surgery because of the high amounts they are on before surgery. I agree with strict parameters for the patients but the other part of the problem are the providers who are writing these prescriptions. I'm shocked to see what my patients are taking when they come to me for an initial consult. Thanks for the comments; these are the discussions I believe we need to have.

In response to Kathy... Yes, suboxone doesnt work for every pt, and it is also has street value. ItRead More is often sold or exchanged for oxy, heroin etc, or used is abused as a rescue drug when one in in severe withdrawal from running out of heroin/oxy. The pill form of subutex is abused via snorting, and is unfortunately available in generic which is cheaper . The Suboxone sublingual form is less abusable in this way. And ofcourse, suboxone/subutex alone without counseling/support in sobriety , and also treating comorbid psychiatric conditions ( bipolar mood disorder, impulsivity, depression, anxiety) that lead to relapse are all very important. Although its not mandated in the State of California, I do a urine toxtst for suboxone and for 12 other drugs of abuse, each time someone comes in for monthly followup appts for suboxone. If they are showing a negative screen for suboxone and positive for other drugs, especially other opiates, then no suboxone will be rxd. If it is an opiate addicted minor ( and sometimes even an adult who is still living with parents), the parents are made responsible for securing and administering the medication. In my practice I often remind parents to lock up ALL medications .. even tylenol etc due to kid's impulsivity/curiosity . Currently am not seeing any new pts for opiate dependence, btw.

My understanding is "Patch" is a site for all issues and I was just posting what I believe is a solution. I served 20 years Active Service in the Navy and am a healthcare professional. If nobody steps up, how do we get things fixed? I'm tired of just complaining about things and not doing anything. Again, I'm sorry if you don't agree with my opinion but I believe this site is just for people who want to voice something.

I would have been interested in the mayor's opinion of the veteran's situation, but the political attacks, that is what I am not interested in. I am sick of politicians attacking their opponents, tired of the badmouthing. If they put as much effort into doing something constructive about problems, like the veteran's backlog of claims as they do tearing their opponents apart then maybe we all would be better off.

I couldn't agree more about if people would take the same effort used to attack others and use that constructively to fix this problem, maybe it would get solved.
